ProMedica Hospice Care- South

Who We Are

Our Mission is to improve your health and well-being

ProMedica Hospice Care provides care wherever you call home: in your house, a skilled nursing center, an assisted living facility or independent living facility. Our caregivers help create individualized care plans, set personal goals, and manage pain and symptoms. Every patient is different. With different challenges, different needs, different family and friends. So the care we give each patient is different. You’ll find HCR ManorCare pursuing this goal at more than 500 skilled nursing and rehabilitation centers, assisted living facilities, memory care facilities, outpatient rehabilitation clinics, and hospice and home health care agencies.
